Pricing, Payments & Refunds
- Prices are subject to change without notice. Taxes and fees may apply.
- Payment is due at the time of service unless otherwise arranged.
- Because services are rendered personally and outcomes vary, fees for services already performed are generally non‑refundable.
- Packages and promotions may include additional terms and expiration dates.
Consent Forms & Pre‑Treatment Screening
Before receiving services, you will complete medical intake and informed consent documentation. You agree to provide accurate, complete health information and to follow pre‑/post‑treatment instructions.
