User Agreement
Last updated October 17, 2025. This agreement covers website use, appointment requests, online booking, patient communications, and access to third-party tools connected to RevIV services.
Use RevIV's website and booking links only for accurate, lawful, personal requests. Do not submit false information, impersonate another person, attempt unauthorized access, or disrupt website function.
Booking a visit does not guarantee that a service, medication, prescription, shipment, or treatment will be provided. RevIV may require intake forms, medical history, consent documents, labs, follow-up visits, payment, and provider approval before care can proceed.
Some medical weight loss, peptide, TRT, and wellness services may be available through telehealth for eligible patients. Shipping availability depends on provider review, pharmacy rules, state law, medical appropriateness, and third-party fulfillment timelines.
Email, text, website forms, and online booking tools may not be appropriate for emergencies or urgent medical problems. Use emergency services for urgent symptoms. By providing contact information, you authorize RevIV to respond to your inquiry and communicate about appointments, services, and follow-up needs.
RevIV may update this agreement as services, technology, laws, or business needs change. Continued use of the website or booking tools after updates means you accept the revised agreement. Disputes are governed by applicable North Carolina law unless another law controls.
