User Agreement

Summary: When you create an account, submit forms, or book services with RevIV LLC, you agree to truthful disclosures, timely communications, and adherence to our clinical policies and instructions.

Account, Booking & Communications

You agree to provide accurate, current, and complete information on all forms and to keep contact information updated.
We may contact you via phone, text, email, or patient portal regarding appointments, instructions, and follow‑ups. Message/data rates may apply.
Appointment reminders are a courtesy and not guaranteed. You remain responsible for keeping appointments and following preparation instructions.

Pre‑Service Requirements

Completion of medical intake and informed consent forms is required before treatment.
You must disclose all medications, supplements, allergies, and relevant health history. Failure to disclose may increase risk and could result in cancellation.
You agree to comply with pre‑ and post‑procedure instructions provided by our clinical team.

Service Expectations & Limitations

Our services are elective wellness/aesthetic services and not a substitute for ongoing primary or emergency care.
Outcomes are not guaranteed and vary by individual. Additional sessions may be recommended to pursue desired results.
We may decline or discontinue services if, in our clinical judgment, they are not appropriate or safe for you.

Payments, Packages & Promotions

Fees are due when services are rendered unless otherwise stated.
Packages, subscriptions, and promotional offers may include separate terms, including expiration dates and non‑transferability.
Once a service has been rendered, fees are typically non‑refundable.

Prohibited Conduct

No unlawful, abusive, or disruptive behavior (online or on‑premises).
No misuse of the Site (malware, scraping, unauthorized access) or impersonation of others.
No false or misleading statements in reviews, forms, or communications.

Privacy & HIPAA

Your health information is protected under HIPAA. Please review our Privacy Policy and Notice of Privacy Practices provided at intake. By receiving services, you acknowledge and consent to permitted uses and disclosures for treatment, payment, and healthcare operations.

Assumption of Risk; Release

You understand and accept the inherent risks of elective procedures and agree to hold RevIV LLC and its personnel harmless to the extent permitted by law for adverse outcomes not caused by gross negligence or willful misconduct.

Arbitration Agreement

Except where prohibited by law, any dispute arising from services will be resolved by binding arbitration in NORTH CAROLINA, with each party bearing its own costs and a pro‑rata share of the arbitrator’s fees. You and RevIV LLC waive the right to a jury trial, subject to limited judicial review as allowed by law.

Changes to this Agreement

We may update this Agreement periodically. Continued use of our Site or services after changes constitutes acceptance.
